WHAT IS MCS-150?
Form MCS-150 is the Motor Carrier Identification Report filed with the FMCSA to obtain and maintain a USDOT number. Every motor carrier subject to FMCSA jurisdiction must file an MCS-150 to register and must update it biennially (every two years) — even if nothing has changed.
BIENNIAL UPDATE SCHEDULE:
Your biennial update is due based on the last two digits of your USDOT number:
• Odd USDOT numbers: Update due in odd-numbered years
• Even USDOT numbers: Update due in even-numbered years
• Due date: By the last day of the month that corresponds to your USDOT registration anniversary month
Failure to update results in your USDOT number being marked "inactive" — which can trigger enforcement actions and affect your ability to operate.
OPERATING AUTHORITY (MC NUMBER):
For-hire carriers transporting regulated commodities in interstate commerce must also obtain operating authority (an MC number) in addition to a USDOT number. Requirements include: BOC-3 process agent filing, proof of insurance (minimum $750,000 for general freight), and a 10-day waiting period after application.

2. DRIVER QUALIFICATION FILES (DQF)
Federal regulations require a complete DQF for every CDL driver. Required documents include: employment application, MVR (motor vehicle record), previous employer inquiry, road test certificate or equivalent, medical examiner's certificate, annual review of driving record, and Clearinghouse query results. Retention: 3 years after driver leaves employment.
3. CSA SCORES (Compliance, Safety, Accountability)
FMCSA's CSA program scores carriers in 7 BASICs: Unsafe Driving, Hours-of-Service Compliance, Driver Fitness, Controlled Substances/Alcohol, Vehicle Maintenance, Hazardous Materials Compliance, and Crash Indicator. High scores trigger interventions and affect your ability to win contracts.
4. ELD COMPLIANCE (Electronic Logging Device)
Most CMV drivers subject to Hours-of-Service rules must use an FMCSA-registered ELD. Track your ELD provider, device IDs, malfunction procedures, and driver training records.
5. DRUG & ALCOHOL TESTING PROGRAM
Required for all CDL drivers. Must include: pre-employment testing, random testing (minimum 50% of drivers annually for controlled substances, 10% for alcohol), post-accident testing, reasonable suspicion testing, return-to-duty testing, and follow-up testing. Must use a FMCSA-registered Consortium/Third-Party Administrator (C/TPA).
6. VEHICLE INSPECTION & MAINTENANCE
Annual inspections required for all CMVs. Driver Vehicle Inspection Reports (DVIRs) required daily. Maintenance records must be kept for 1 year + 6 months after vehicle leaves your control.
